Dear Sir/Madam, My name is Manoj Kumar. I have recently obtained an Aadhar Card for my mother. Unfortunately, the date of birth was printed incorrectly. There is no Date of Birth Certificate available for her (as she is very old and such certificates were not in use at that time). I contacted UIDAI customer care, and they informed me that I will need a Date of Birth Certificate from a Class-A Gazetted Officer on his letterhead to get the DOB changed in the Aadhar Card. However, I could not find a proper format on UIDAI or related websites. I would greatly appreciate it if someone could provide the correct format to me.
