I am working in a Pharma company as an HR Executive. Our management has announced to give an increment to all the employees effective from Jan-16.
Our MD sent an email to all the employees stating that everyone needs to send their self-appraisal containing their achievements, etc., to the MD through email only.
Now my question is how to send an email? Please share your valuable responses along with a draft email.

When seniors ask juniors to submit reports, whether for self-appraisal or any other purpose, it is their duty to provide the format of the report. Your MD has done half the job. Now, managers could send their self-appraisal report as per their understanding, and it would take a whole lot of time for the MD to analyze the information, and it will not be in a standardized format.
Whether through mail or in hard copy, what matters is a standard format. Standardization is a step towards time management.
Even now, you may talk to the MD and tell him to send an email stating that the self-appraisal format will follow shortly. Prepare the draft of the self-appraisal, show it to him, and let him approve it. Once approved, either let him send an email to all the employees or you send it on his behalf.
Asking for certain information but not sending the format of the information is a typical mistake that happens in the private sector. In contrast, in most government organizations, seniors attach the format of the report or return it.
