Dear friend,
I shall reply to your queries.
1. Employer and employee have to contribute equally to EPF. But out of 12% of employer contribution, 8.33% will be paid to pension fund. Rest of the amount (3.67%) will be credited to EPF. That is why the difference in these contributions.
2. For withdrawals from PF and pension fund, form 19 & 10 C (respectively) are used.
3 & 4. Apply thr' concerned employer

Agreed with Abbas.
Regarding your querry please confirm whether you have completed 06 month service or not?
If not then you can not withdraw your EPS part. You will be eligible for PF part only.
