Good Morning,
I work for a facilities management company and I have been asked to prepare performance appraisal forms for the following designations;
1. Facilities Manager
2. Accountant
3. Contract Manager
4. Billing officer
Please, I need help in preparing these forms. May I have samples of performance appraisal forms for these designations?

Performance Appraisal (PA) forms do not vary according to the designation. PA form is uniform is across all the designation. At best, you can have one form for managers and other form for non-managers.
PA form is part of your Policy on Performance Appraisal. This policy itself supplements Policy on Performance Management System (PMS). Therefore, first you need to establish well-designed policy on PMS. Policy on PMS apart, you need to train your staffs on designing KPIs/KRAs or differentiating between these two. I recommend you doing this spadework first. Any half-hearted job or piecemeal work could do more damage than good.
I provide consulting services on establishing comprehensive PMS. I study the systems process of each department, find out direct and indirect costs of each department and design KRAs that are centric to these costs. Please confirm whether your management is ready to avail of my consulting services. Be assured that it will bring sea change in your organisation.
