FYI

One fatal accident happened at PGCIL (Power Grid Corporation of India Limited) (Contractor - Jyoti Power) Aurangabad Stringing work site, in which 4 workers died and 2-3 workers were seriously injured. This accident happened during the final sag operation, in which the dead-end tower (nearby gantry) collapsed and four workers who were working on the tower were crushed between the collapsed tower.

Following are the probable causes of the accident:

1. Backstay not given to the dead-end tower.
2. Over-sagging resulting in the collapse of the tower.
3. Final sag operation was carried out with two tractors.
4. Winch machine usage was not in operation.
5. The tower was not fully tightened.

Preventive Measures to stop this unsafe condition:

1. Backstay shall be fixed at least 1.5 times (height of the tower) before the start of stringing work.
2. The depth of the dead man pit should be 6 feet, and the minimum diameter of the dead man wood should be 12 - 14 inches.
3. Double backstay arrangement may be given to strengthen the backstay.
4. A minimum 18 - 20 mm diameter steel wire rope shall be used for the backstay lead.
5. Final checking of the tower must be done before the start of stringing activity.
6. Use of the winch machine.

Therefore, please take necessary precautions during stringing work and suggest the same to the team. This is for your kind information and necessary action, please.

Dear Sir,

I need to know the cause of an incident that happened at my site. The oxygen regulator was suddenly sheared off from the cylinder with a heavy sound. I also found both the regulator and flash back arrestor of the oxygen side in a burning condition. There was no hot work being done above it. Luckily, there were no injuries or need for first aid.
