1. I am assuming that you are asking about group health policies.
2. In a group policy depending upon the size of the group
a) you can either go for a seperate sum insured policy or a floater policy.
b) you can cover only employees, employees+spouse+children, empl+sp+Ch+parents.
c) you can cover pre-existing ailments and maternity benefits.
