I need some help here. I need to prepare a template where all the directors of our organization need to give a performance review for the CEO. This is basically called reverse appraisal at our organization, where juniors appraise their seniors. I need to prepare a form in the form of Q&A (objective as well as subjective). Can anyone suggest to me what all can be the contents of this?

What you are referring to is a 360-degree form of appraisal system. This system should have a questionnaire related to "Management Ability" of the seniors which the juniors feel is a well-known trait of the bosses/seniors. Management Ability would mean - How does the person react to a situation? Are qualitative/quantitative techniques applied? Interest in resolving issues/conflicts, ensuring smooth running of the department on one hand, and planning, delegating, forecasting, budgeting as major traits. Design the few things mentioned above bearing in mind the level of the person is in.
