Hi Experts, I had quit my previous job in June 2007 and had applied for a PF transfer from my new company. Even after 4 years, it was not transferred, and I was not able to track it. So, I decided to withdraw the amount and applied for the same. The form was submitted from my previous company with the company seal to the PF office in Bangalore. However, the application got rejected, asking me to submit the forms again with an indemnity-cum-affidavit letter.
If I want to apply for withdrawal again:
1. Can I submit the same forms directly to the PF office with the new affidavit letter, without going through the company? The application that was rejected was sent back to me with a seal on it from the PF office.
2. Is there any other letter needed stating the reason for not withdrawing the PF for more than 3 years? What's the format for that?
Thanks,
Sinu

Again, you need to fill out the withdrawal form with your previous company's seal and authorized signatory, along with an affidavit letter. I will send you the affidavit format. However, you will need to go to any advocate's office and inform them about the case.

PF Withdrawal Process After Leaving an Organization
After leaving the organization, you will have to claim the PF within a period of 1 year. Otherwise, you must submit the PF Withdrawal Application Form 19, Form 10C, a copy of the first page of your Bank Passbook, Name Proof/Address Proof, PAN Card Copy, and the experience certificate/service certificate issued by the previous employer in which your PF Number is mentioned, along with an Affidavit letter duly attested by the Notary Public.

As Mr. Bhaskar suggested, Form 19, Form 10C, Bank Passbook first copy, or a cancelled cheque leaf should be enclosed. However, address proof and a PAN copy are not required. In fact, an Indemnity Bond-cum-affidavit must be submitted. It is important, once again, that the authorized signatory from the previous company attests. Please ask them for Form 5 and 10 along with Form 3A for the full period. This is crucial because, in Bangalore, especially in the KR Pura PF office, there is no track of the documents that employers submitted.
