Unauthorised Absence

1. You had asked for leave for ___ days from ____ (date) to ____ (date). Your HOD, Mr/Ms ____, had sanctioned it. You were supposed to report for your duties on ____ (date). However, you reported for your duties on ____ (date).

2. Your absence from ____ (date) to ___ (date), i.e., for ___ days, is unauthorized. During your absence, we did not receive any communication from you to justify your overstay.

3. This unauthorized absence of __ days is misconduct under the provisions of Section ____, Paragraph ____, clause ____ of the approved and certified standing orders.

Please fill in the blanks and print this on your company letterhead. Obtain the employee's signature on the second copy. Ensure the second copy is also signed by the authorized signatory, and do not use a photocopy of the original.
