Dear Mannubhai,

Here, I am attaching a sample of the daily shift report format that we use in our company. It is filled out shift-wise, and a combined report is circulated in the morning to all for corrective and necessary action. You can modify it as per your plant requirements.

We have a citation book in 3 copies whose contents are as follows:

Company's logo & name
Unit:
Date & time:
Name of person:
Gate pass no./Emp. No.:
Subcontractor:
Nature of safety violation:
Plant/Location:
Action Taken:
Remarks:
Signature
Name of safety officer

This format is filled out in three copies: one is given to the person who is imposed a penalty, one slip is sent to the finance department to deduct the money from their salaries or contractors' work orders, and one is kept in the safety office for record-keeping.

On a monthly basis, we compile data and send slips to finance for the deduction of money. A compilation sample report is attached.

Dear Abhay,

It depends on the nature of the violation. If it is not serious, then we give an oral warning. Otherwise, we directly deduct it. For example, if a helmet is lying near him and he is not wearing it, we warn him orally. If he wears it, then it's okay. However, if we visit again and he is not obeying, then a written warning will be given. If he does not have a helmet at all at the site, then penalize him.
