A couple of points. I do not know what the objectives of your project are. Hence, I am only going to comment on the questionnaire layout. It's better to ask personal/demographic questions at the end. It's also better to use a web-based facility for conducting the survey.
I noticed that I could only open the document in "Read-only" mode. While this allows for answering the questions, responders have to save it, I believe, and then send you the file (once you provide your email address). Do you not think you are expecting a lot from busy people?
It is also advisable to clarify the purpose of your survey to the responders.
