when u r leading a safety meeting, then make a presentation by focussing on the following points -
1. Total accident free hours YTD
2. All accidents & near misses YTD statistics
3. Additional safety information of your company
4. Status of action items on the previous meeting points
5. Latest safety news around the world.
I cannot send a sample presentation. but u can have this as a base & continue with this. this will be for presenting the meeting. apart from this, after the meeting u need to make the MOM & circulate it.
ur points are too gud & elaborative. i think, urs also cover the same in breif. to be frank, i have downloaded ur file & need to go in detail.

First of all, I think there will be no problem to you in sharing such document of your company in which even names of your top executives area there...
Secondly you may add Agenda of meeting after the attendace part of your MOM... This can give an impression of what all things you have discussed in meeting even though if there no point arousing which to be noted in MOM...
One more point from MOM is that you can avoid the highlighted by column... This makes no sense as it's understood that point to be discussed arise by any of the forum member...No use by specifying it upon MOM, only result is the key area where we need to focus... If any specific reason behind it please let me know the same...
