Dear Anshu, first one - she can nominate second & third one - if employee married / unmarried she can mention her family details. regards, suresh

Part A, the member can fill in the percentage mentioned clearly.

Part B1 is usually for a spouse and children; however, unmarried individuals can fill in the parent's details.

Part B2: If there is no specified family member (i.e., B-1 is blank), namely parent, wife, or children, then the member can nominate a brother, sister, or any other blood relative with their details.
