I have one doubt regarding the name change in EPF records, in our concern one employee told her pet name to us when she joined we recorded that name as her name and we deduct EPF and ESI from her salary and paid that amount to the pet name, now she did not have any proof for her pet name, in that case how we would help her to claim her EPF amount please give suggestion.

The link given above will not help you.

If there is a minor mismatch of spelling in the name or mismatch in date of birth then one can rectify the same by uploading joint declaration but in your case their is a major mis-match in the name.

The person has to prove that the both names i.e. the actual name matches with aadhar document & other documents and the name registered with PF i.e. the pet name, are belongs to same person and one person only. He has to get it certified from court on affidavit. The PF department will effect the change on such document only.

How you accepted the name i.e. the pet name of the employee? That means you are not obtaining Form 11 from the employee at joining. You are not verifying the identity of the candidate on his joining. You are practising Lallu HR Practise. Isn't so?
