Dear Ranjeet,
According ESIC sec 2(22) refers to defination of wages which means any cash paid towards salary shall be deducted for ESIC. It may be LTA / Medical or other wich included his gross salary.
Remember as thumb impression that employee drawing gross salary of Rs.15000/- shall be covered under ESIC and above shall be excluded.

If any payment paid on monthly basis on any head it will attract ESI coverage, if the gross is less than 15000/-. But the same was paid or reimbursed once in three months then it will not covered under ESI.

As per ESIC Act any by monthly payments made to the employee can be consider as Wage / Salary.
LTA we can pay LTA on monthly basis and for tax benefit we can Fix one Month basic or any xyz value as per IT Act.
That will help u to resolved u r problem
