Thanks for posting the questionnaire. I liked it. However, here are a few tips.
I would have much appreciated it, had you given some more information about who you are, where you are working or studying and what's the final purpose of the survey. Also, it would have helped you had you stated how you would reward people who spend time to answer your survey; for example, give them a copy of the summary of findings.
The questionnaire would have been shorter had you used a tabular form with columns for the responses rather than repeating the scales after each question. I have made this comment for the benefit of others, too. Finally, a question: did you pilot the questionnaire before posting?
I very much liked the table defining the scales used.

Hello Everyone,
I am doing my MBA dissertation. Actually I work in an IT company, and i have choose the topic of my dissertation from my place of work.
The problem, at my comapny is that the key staffs(knowledge workers)from both Infrastructure and applicatoin department are leaving who have severals years of experience. When these people leave they take their tacit knowledge(their own way of doing things gain with a lot of experince) with them. Our comapny loss interm of knowledge and the managment has to invest a lot to train new staff.
My objective will be to find a way how to capture the tacit knowledge from these experts before they leave the comapny.
If anyone of you have any questionnaire, projects or any notes to share with me.. please feel free.
