There are two slabs:
For Employees drawing wages upto & inclusive of Rs.3000/-PM
EEContbn-Rs.6/- per employee
ERContbn-Rs.18/-per employee
Employees drawing wages exceeding Rs.3000/- PM
EEcontbn-12/- per employee
ERContbn-36/-per epmloyee
EE(employee) Contbn can be deducted from employee's salary
Contributions are payable twice in a year (For Half year ending JUNE & December of every year)

Jay,
I am attaching a jpeg file of the form cum return, which is self explanatory.
However a word of caution to all who will be using to file their returns and contributions for the first time.
We are in a duel with the authorities as they interpret designations and job description as per their thinking, therefore if you decide to include officers/executives as supervisors when filing for the first time please have a document written somewhere and place it in the mlwf file so that anyone in future can read this carefully, else when your workforce grows they will aks you to include officers as workers, so be careful.
EEC - Employees contribition
EER - Employer contribution

IN U.P. their a Labour Welfare Act. but in this Act their is no regular contribution to e paid only if any employee left with taking his full payment his part of payment ( unpaid wages / salary ) is to deposited
under this act.

in Haryana - Labour welfare Fund contribution is 15/- per month per employee( 5/- employee contribution + 10/- employer contribution ) w.e.f 1st April 07. regards

Hi Jay...
LWF applicable only for workman category as it need not to be collected from executives. but again it is the sole discretion of the Management as whether to collect the money from every employee towards Labour Welfare Fund.
As far as Karnataka is concerned..
Employee Contribution is 3 Rs.
Employer Contribution is 6 Rs.
To be submitted along with Form V

Labour Welfare in DELHI:-
As per the provision of the said Act, any shop, establishment/factory, which employees five or more then five persons on any working day during the preceding 12 months, owes a statutory obligation to deposit: -
a) Fines realized from employees,
b) Unpaid accumulations,
c) Contribution of employees @Rs.0.75 per employees, per six month.
d) Contribution of employer @Rs.2.25 per employees per six month.
