The Mumbai Labour Welfare Fund Act, 1953
I. Period of Deduction
Contributions is to be deducted in respect of the employees who are present on the muster roll as on 30th June and 31st December of Every year.
II. Last Date of Payment
For 30th June ending .... on or before 25th July
For 31st December ending .... on or before 25th January
III. Rate of Contribution (W.E.F. June 1993.)
Employee’s Contribution Employer’s Contribution
Salary up to Rs. 1000/- 1.50 4.50
Salary from Rs. 1001/- 9.00 9.00
and more 3.00
Managers are not covered under the Act and no contribution is payable in respect of Supervisors drawing salary above Rs. 2500/-.

I have the formats of the Registers of unclaimed wages to be maintained under the rules of (Gujarat and Delhi) Labour Welfare Fund, but I am not able to find out the format of unclaimed wage register under the Bombay Labour Welfare Funds Rules, Can anybody help me out for the same ?
