First of all, there is no break in service certificate as such prescribed under EPF&MP Act 1952.
As far as PF Account is concern, there is no relevance of break in service. Whatever amount is accumulated in your account, you are liable to get it with interest. In-operative account is a separate issue.
However, in Pension Account, break in service is relevant factor. In Pension From 10C, employer has to write non-contributory service at the end of the form and in From 10D also at the end employer has to certify the non-contributory service during past 12 months of date of leaving the service.
There is no need of separate certificate of break in service. But still some offices insist for such separate certificate and we give. We don't go for confrontation with the department.
I have developed one certificate in word which is attached herewith. You can use the same or the one which is given in excel by our learned member Mr. Janardan.

Dear All,
The Break In service ( Non Contributory period where no contribution is paid to EPS ) is to be certified by the employer for 10D and 10C application.
The EPS settlement is done on the basis of average of last 12 months salary * no of years service as per table D.
If due to the break in service the service becomes less than 6 months, the year is not considered for service for pension ( 10D and 10C ) purpose.
If there is break more than 180 days, the total service will be reduced for one year.
Many times in case of female members, for maternity purpose they take leave without pay or in other cases due to medical or any other reason if there is non contributory period, it is expected to record the same with employer.
Employer gives certificate in the form 10D for non contributory period.

Why this is asked by epfo dept. to issue service break certificate. Even we have sent all ncp days detail in 3a form up to date 28.02.2012 and then after sent in ecr files.
if person has employed with us from last 20 or more then is this not a typical task to make his service break certificate.
I think this has become irrelevant in nowadays.

my Clint PF is rejected => Claim submitted Rejected due to -> AS PER OFFICE RECORD ECR NOT RECEIVED FROM 01/2018 TO 05/2018 AND ALSO NCP DAYS IS NOT GIVEN IN LEDG/AS PER OFFICE RECORD ECR NOT RECEIVED FROM 01/2018 TO 05/2018 AND ALSO NCP DAYS IS NOT GIVEN IN LEDG
so what i do in this case also EPFO need the back certificate so give the any format
