Questionnaire regarding the welfare activities in the company:
1) How many workers are working in your company? If more than five hundred during the preceding year, have you employed a Welfare Officer?
2) Are more than 1000 workers employed in your company? If yes, have you appointed a safety officer?
3) Have first aid boxes been provided in case you have more than 150 persons?
4) Are you giving a creche facility to the lady employees? (As per the Factories Act, 1948, Creche facility is provided where 30 or more ladies are working).
5) Is a canteen facility being provided by the company in case you have 250 employees or not?
