The transfer of funds is online now, and there is no significance for form 13 (transfer of fund from one PF to another) or form 3A(Contribution card). But if the old account is not linked with Universal Account Number, the offline forms can be used. Whereas the form 3 is to be given by the employer only. Normally, a photocopy fo the form 3A submitted by the employer along with form 6A (annual return) is attached for easy perusal by the EPFO. If not available, you have to prepare it with the details called for. PDF of Form 3A is attached.
