I am barkha working in a manufacturing firm of stitching. As i want a format in which operator or stitcher have to fill the details of machinery on/off details. So many operator leave the premises without shutting down their machines due to which their is chances of severe miss happening as our firm is from stitching line. so kindly suggest me a format which have authenticity.

The ideal way to control this is to keep a log book for every machine and the User should enter the ON and Off time in the log book. However even then there are chances of Users missing to enter the details. Hence I suggest to keep the log book in the Time office / Attendance bay so that Time Office Supervisor / Security Office can monitor /instruct to enter the details without fail.
