Please confirm whether it is required for the State Govt (EDist. Portal) or Central Govt (Shram Suvidha). The major documents are as follows: Login details of the authorized representative (Aadhar), Client agreement (showing period), Form V, Declaration Certificate with MD/Proprietor ID Proof (Aadhar, PAN, etc.), Address proof of the representative, Contact number, and Email details of HO-Site Representative. The size of all the above documents must be less than 100 KB.
