Firstly, who is the employee, and who is the sanctioning authority? What is the big deal in drafting an email for yourself on this? Leave applications are generally submitted through a hard copy with a recommendation for approval from the employee's supervisor, followed by the approval from the Head of Department (HOD). Even if it is via email, a request can be made with the necessary content and dates, which should suffice.
