Please suggest forms to be used for a change of occupier position from one director to another director. The old occupier will still continue as a director. This is most urgent.

PF Authority - Form 5A
ESIC Authority - Form 1 - which is now available online in the company's ESIC code account.
Factory License - as per the local government authority (consult the factory inspector's office).
Professional Tax - As per the Municipal Corporation or state government - whichever applies in your state.

First of all, please obtain a certified copy of the Board Resolution passed by your company's board in which the decision on the change of occupier was taken. Then send a copy of the same to your area's Director of Factories, intimating about the change.
Thereafter, please send Form No. 4 (the number may vary; as per UP Factories Rules, it is 4) in triplicate.
Just go through Section 7 of the Factory Act, and you will have an idea about it. You have to deposit the prescribed fee for this (as per UP Rules, it is Rs. 5/-) along with the form, a copy of the challan of the fee so deposited, and the original Factory License. These should be deposited with the Director of Factories of your area for making the changes after approval.
Please note that not every director of the factory can be nominated as an occupier. Only the director who has ultimate control over the affairs of the factory can be nominated, so please take precautions in this regard while dealing with the matter of change.
