Request for Assistance in Implementing a KPI System

Please help me with the paperwork on how to implement a key performance indicator (KPI) in a company. My employer wants me to develop a KPI system to evaluate staff performance so that we can measure their performance level. This measurement will be used for increments, designation reviews, etc. Please share your paperwork on this system because I have no idea how to create and implement a KPI system.
