I went through your dining etiquette details. Good! In fact, for the novice or an amateur, it is very handy.
Though, I would suggest you also work out a detailed format for the table manners and the use of forks and knives. This should include the different kinds of forks and knives and their various uses, the use of a serviette, the correct placement of the wine glass, the water glass, and the like.
It would be more helpful and improve your write-up as well.
