My name is Prachi Srivastava, and I am a management student specializing in HR from Alliance Business Academy, Bangalore. My course will be completed in June 2007, and currently, I am working on my dissertation. My topic is to study attrition in BPO and propose a retention model for the same.
I need to collect data from employees who are presently working in BPO. The help that I require from you all is that I am totally clueless about how to prepare questionnaires for data collection. Please, if possible, send me some sample questionnaires. Additionally, I need assistance in getting these questionnaires filled.

Dear All,
Questionnaire for persons who have stayed back in BPO for more than a specified amount of time. Another for those who left within less than the expected time frame. Based on these two types of data, you can discover reasons and maybe evolve a policy to take care of both groups.
As for the ready-to-eat questionnaire, I'm sorry, but you have to search the net.

You can prepare a questionnaire based on parameters such as:
1. Questions about salary (you can specify as compensation).
2. Questions about work pressure.
3. Questions about flexible work systems.
4. Questions about any refreshment facilities available in the company.
5. Questions about traveling facilities given by the company.
6. Questions about motivation given by team leaders of the employees.
7. Questions about training given at the time of induction.
8. Questions about infra facilities available in the company.
9. Questions about the satisfaction level of the employees.
10. Finally, suggestions to improve the current situation.
You can use open-ended questions for easy understanding of employees. If any help needed, I will ask my friends.
