PA is a complex subject and has to be designed according to the type of business or the type of people you are trying to access. For example, assessment in the manufacturing industry will be different from accessing retail business employees. Also, it depends much on the category of employees you are accessing. For instance, SALES, ENGINEERING, COMMERCIAL employees will have to be accessed according to the duties they are expected to perform. One of the methods of performance evaluation is by assessing achievements against set objectives (MBO); another is accessing behavioral expectations of a particular employee, his attitude, etc.
It is difficult to give you an entire procedure over this forum, but based on the tips I have given, you can develop suitable assessment forms on your own. It is very interesting and challenging. Hope I have helped a little.
