Hello Aditi,
I am a retired academic and was interested in just checking what it was about. I did not find any information on what it is all about and what you want people to do with it. It is always advisable to introduce yourself and the questionnaire (if that's what it is), and also say why respondents should fill it, what's in it for them, etc. For example, offer a copy of the summary of my findings.
