What kind of personal information should I ask of employees as an employer? Management asked me to get information as the given format.
Is it legal to ask for such kind of personal information, and is there any legal provision to avoid such kind of information?

It is not the legality but the objective which is more important. As an employer, you can ask his personal details which are relevant for the relationship between you and the employee. You need to know where he is coming from, his residential address, locality and even ask about the father's name or any person who can be contacted in case of emergencies, if something happens to him during the course of employment. But what is the relevance is asking the details of his uncle, aunt or grand mother or in laws? That should be avoided not because it is illegal but because it is not relevant for employment.
