In this case the forms are very much available you have to get a certificate from the employer and self certificate, and both the forms need be submitted to the ESI authorities for getting the benefit. You have to get a declaration from the medical board to proceed further..I think you are having disability to continue your service further, as is the case you have to produce related documents for getting the certificates from the employer... if the disability was happened during the service the process certainly is easy to do.
