As Mr. Dilip mentioned, proper accident investigation techniques vary from place to place and from company to company. Even though it may differ between individuals, the fundamental concept of investigation remains the same.

    Dear Raghu,

    You are my type of man - collecting different useful material from different sources and keeping them in a library. I do the same as well. You never know when they become useful.

  • Dear Mr. Pillai,

    The MISCTRIPOD ppt is excellent. In slide numbers 20 and 21, you have used the abbreviation TRC in the triangle comprising Empathy, Culture, Motivation, At-Risk Behavior, Near Misses, TRC, LTI, and F. Could you kindly explain what TRC represents?

    Similarly, in slide number 69, you mentioned PAER. What do these letters mean? I work in an Engineering company, and these terminologies are new to me. I appreciated the entire presentation, and I noticed that some concepts of FTA and ETA are used. The example provided about burn injuries made it easier to understand.

    Dear Sudhir,

    TRC stands for Total Reportable Cases. PAER - P for people; A for asset (or property); E for environment; R for the reputation of the organization. These are the four main areas that are affected by the occurrence of an accident or incident.
