I would like to take a loan against my PF. I am already 56 and a half years old and would like to withdraw 90% of the contribution of my PF (employee + employer contribution). I hear that I can withdraw this without giving any reason for the loan. Is this correct? If so, what is the form number I have to submit, and what reason for the loan needs to be specified by me?

You may withdraw up to 90% of the amount standing in your PF account within one year from the date of superannuation without specifying any reason. However, the minimum age to avail of this withdrawal is 54 years, and it is to be applied for in Form-31.
