We have issued an offer letter to one of the prospective employees in our organization. We need to revise the terms and conditions (specifically related to salary) and issue a fresh offer letter. Could anyone help with a standard format stating the revised terms in a new offer letter that explicitly states the previous offer letter is now void?

Normally, the letter of intent does not carry the terms and conditions. It states terms & conditions as mutually agreed, and the appointment letters shall be given at the time of joining the duties. If you have to correct it, you can always refer to the previous letter of intent to change the terms, but that should happen before joining; otherwise, it is challengeable in the court of law. I shall send you the format by evening.
