One of the female internal employee in my organisation is going for her maternity leave. What are the forms that she would need to sign? Any other formalities that needs to be done? Kindly suggest

There is no specific format specified in the law. The only thing a female employee needs to do is inform the employer within the stipulated time frame in writing, along with medical reports, to avail the benefits under the Maternity Act.
