Please find below my answers to your questions. I have one suggestion: you should convert this questionnaire into some kind of objective type. For example, ask participants to give a rating on a scale of 1 to 5 or something similar. People who are willing to answer may not have the time to write detailed responses. Even if you receive a lot of replies, people may answer very differently, and collating them would be a stupendous task.

Many corporates use Training Needs Analysis as the basis.
4. Where can soft skills be acquired?
Professional consultants offer different courses. There are in-house programmes in some companies. Some individuals try to acquire them on their own through self-study.
5. Can personal traits and habits be changed through such skill sets?
Certainly, if the individuals are willing to change.
6. During an interview, are soft skills considered as crucial as technical/Hard skills?
Very little time is devoted to soft skill assessment.
7. Is there any test conducted during interviews like a psychometric test?
Selectively done.
