My views - Your efforts and interest are really appreciable. However, I personally feel its too lengthy and subjective. Can make it more userfriendly.

Some of these questions are too personal, I think. In some companies, it looks like they want you to leave because the culture and environment are not up to standard. So, when you are leaving, what do you say if you love the company? I need some input on this idea. When we leave a job, it is because we are not satisfied, right.

Hi Vina,

The questionnaire can be redrafted the way you want to put the questions. If we ask a person to leave the job or if he resigns himself, we just try to find out ways to overcome any repetition of circumstances for those who are working. At least we can do some homework and improve our processes.

Since this form has been introduced in my company, the only way people leave the job is out of their poor performances as we have tried to overcome all independent variables. Many times people leave you in doubt, but many times you get surprising answers from them. So such questions should be put in very diplomatic ways and not exactly the way they are written.
