Hi. As I understand the subject, the questionnaire that you are looking for will have to be made by you, and there cannot be any ready-made solution. You will have to catch hold of their Job analysis, PIQ (Position Information Questionnaire), and then see the linkages drawn to their PMS (Performance Management System) with the appropriate weightage drawn, if any, since there will be not any one but a set of competencies forming the JS (Job Description).
