There is no specific format for marriage leave. It can be a written request to the reporting authority seeking sanction with a support document, namely , wedding card for the records and reference.

Formats keep changing based on organizational requirements. The standard information required in a leave format includes:

1. Date of application
2. Addressed to the sanctioning authority
3. Period of leave: from & to
4. Reason duly signed by the employee in case of a written request
5. Provision for the sanctioning authority's signature

Currently, many companies are using software for time office and payroll, which helps in updating employee data.

Hi,

Various types of leave may include Annual leave, Sick leave, Compensatory leave, Educational leave, Casual leave, etc. The form may contain all of the above or whichever is suitable for your organization as the title. Then the details such as:
1) Employee's Name,
2) Employee No. if any,
3) Department Name,
4) Employee's Designation,
5) Leave From: _____ To: _____,
6) Number of Days,
7) Resume duty on: _____,
8) Leave Address or Phone No.: _____,
9) Employee's Signature & Date,
10) Approving Authority - Signature And Date,
11) For office use: Sl. No. if any, data posted by, etc.

If you try to create a form based on the above details and even make improvements, it would result in a better Leave Application Form. I hope this is useful to you.
