Allotment of ESI code is regional. Therefore, it is better to get a separate code number for the branches in different states. This is required to provide medical and other benefits to the member employees since they will be associated with the local office of their branch only when they have a separate code number corresponding to the Regional Office of the ESI.

The simple procedure to obtain ESI's subcode is to write a letter with the address of your branch and attach a copy of the original code. Then, send this to the regional office of your branch. You should receive the subcode within 15 days.

It can also be obtained instantly if you send someone from the office or go personally to the nearest PF office. Normally, they entertain these kinds of queries until 3:00 PM. I'm not sure which region you are looking for.

For applying ESI sub code, you will get a form from the ESI regional office, and you have to submit the documents listed below:
1. CIN form
2. Memo of Article
3. List of employees of that particular branch covered in ESI with Name, salary details, Designation & contribution.
4. Agreement or any contract (Depends on the nature of work)
5. Lease agreement
6. Demand draft of 6 months' contribution in advance in favor of ESI if you are a placement consultancy, manpower supplier, or security agency.

I have the main ESIC code in Kolkata for my organization. I have deployed 5 to 6 employees in West Bengal. I do not want to go for ESI subcode for these locations to avoid separate half-yearly returns and payments as we have 36 main codes in PAN India. Is there any notification from ESI that without the subcode the employees employed in the local area can avail ESI benefits? Please revert.

There is an initiative to allot biometric numbers to insured persons and their families so that they can avail medical facilities from any of the ESI dispensaries in India. Once that has been done, to a great extent, the problem could be solved. Until then, a sub code will be required.

For ESI Sub code, just visit your respective ESI Office and ask for ESI Sub Code generation. They will provide you with an application form for the sub code. Simply fill it out, attach an attested copy of form no. C-11 (this is the main code for ESI of your Branch), and also include a list of employees of your establishment along with their joining date, monthly salary, etc. Additionally, take the rent deed of the establishment (sometimes it is required).
Then, submit all the documents to the ESI officer. Within 10-12 days, you will receive the ESI sub code for your branch. It's simple.
