Please help me out with the procedure for filing the annual return of bonuses. Which form needs to be filled out? Where should this form be submitted, and how should it be filled out? Your assistance in this matter is greatly appreciated. I am part of a Chemical Manufacturing company.

Bonus Annual Return Form D Submission
The Bonus Annual Return Form D is to be submitted within one month after the expiry of eight months from the end of the financial year. It should be submitted to the Labour Commissioner's Office of your region.

As rightly said by Smitha, it is Form D under The Payment of Bonus Act 1961. It is a return of the bonus amount that has been disbursed and the number of employees who are eligible to receive the bonus.
Herewith, I have attached the Form D for your reference. It has to be submitted to the Labour Commissioner's Office/Assistant Labour Commissioner's office in your area. Please keep in mind that the statistical data should exactly match the data of the actual paid bonus and the data in Form D. It must be submitted before the 31st of December of this year.
So, hurry and submit the same at the earliest.

Sec. 27: Inspectors - (1) The appropriate Government may, by notification in the Official Gazette, appoint such persons as it thinks fit to be Inspectors for the purposes of this Act and may define the limits within which they shall exercise jurisdiction.
According to this section, the Inspectors under The Payment of Bonus Act, 1965 may be the Government Labour Officer / Assistant Commissioner of Labour / Commissioner of Labour as notified by the Government. (If we consider the declaration for Maharashtra State, the Government Labour Officer / Assistant Commissioner of Labour / Commissioner of Labour are the 'Inspectors' for the provisions of the Bonus Act, and they have powers to inspect and verify the records related to Bonus.)
Usually, we call them Labour Officer / Commissioner rather than calling them 'Inspector.' Hence, I have stated that the return in FORM D should be submitted to the Labour Office / Assistant Commissioner's office for that region.
