Dear Colleague, The nominee under the EPF/EPS can claim PF, EDLI, and EPS online through the employer where the deceased employee last worked.

Document Required

A) Death certificate, in case the application is by the nominee/surviving family members or their guardian/legal heirs after the death of the member.

B) If a Nomination Form is already existing in the name of the family members, then a copy of the Nomination Form. If it is online (E-Nomination), then it will be available on the EPFO Portal itself, and there will be easier settlement. If there is no valid nomination, then a Legal Heirship Certificate issued by a Revenue Official. If there are no valid documents mentioned like Nomination/Legal Heirship, then a Guardianship certificate issued by a competent court of law, if the application is forwarded by a guardian other than the natural guardian of a minor member/nominee/family member/legal heir.

C) Cancelled cheque so that the payment may be sent through ECS in the claimant’s account.

D) First page of the Bank Pass Book in any Nationalized Bank listed by EPFO.

D) EPF Form 5 IF for claiming the benefits under the Employees’ Deposit Linked Insurance Scheme (EDLI) if the member dies while in service.
