Can any one help me with the format of: Death Certificate while in service EPFO says that the above is to be issued by the principal employer.

Issuance of Death Certificate
The death certificate is normally issued by government authorities such as the Municipal Corporation. As an employer, we are not supposed to certify the death of an employee as it would not be legally binding. Other certificates, such as the Legal Heirship certificate, can be obtained by applying to the Thasildar, which are valid. As an employer, legally, we are not bound to fulfill this requirement.
