Is he eligible for any kind of pension under the EPF Act? Also, could you please advise on the specific PF form that needs to be filled for this purpose?

In my view, if the competent authority of the Health Department issues a certificate of Total Permanent Disability, only then can the person receive a pension under EPS 1995. Please also contact the ESI authorities for benefits under the ESI Act in such cases.

On the death of a member while in service, the family/nominee will receive a pension even if they have contributed for only one month to EPS/EPF. Please search CiteHR for details of forms. I had also replied to a similar query with details of forms and documents required. If you don't find it, please let me know, so I can provide you with the details.
Please note that the widow and children below 25 years will receive a pension. According to the new rules, which are yet to be implemented, the minimum pension will be more than 1000. The pension entitlement of the widow on the death of the husband while in service will be higher than what the employee would have received as a pension if he had retired on the day of his death. The widow/nominee is also entitled to an insurance (EDIL) amount, which is built into the pension scheme.
