Performance Appraisal Suggestions Needed

I am working on Performance Appraisal and preparing appraisal forms as well. Need your inputs on the below:

- How can effective performance appraisals be conducted (e.g., quarterly, bi-annually, or annually)?
- What are the main points to be added in the appraisal form?

Performance appraisal should be a continuous process. Appraisals can be conducted monthly, quarterly, or yearly—it depends on how big your organization is and how much importance management places on performance measurement. You can opt for a yearly appraisal, but my suggestion is to track employees' performance each month. Consider using the critical incidents method, where the supervisor notes critical incidents of subordinates in a diary for reference during the yearly appraisal.

Yearly performance appraisals can be based on three parameters:
1. Output
2. Behavior
3. Attendance
