The name of the PF account holder is incorrect in EPF records, and now the PF account holder is deceased. The claim has to be submitted by his wife. My doubt is, should we fill out Form 20 D with the correct name as in the ID cards, or should we fill out the form with the name in EPF records and provide an affidavit for the correct name and the relationship with the deceased?

I think it's 10-D, not 20-D. Here, there are two types of processes you can follow:
One:
You can fill out Form 10-D as per PF records and get a signature from the PF member (if he can't sign, there is another process) to get the PF amount released. The bank account name should match the PF records name; otherwise, the money won't be transferred.
Second way:
You can meet the company HR and apply for a name correction on the PF records. The company HR will write a letter to PF officials, and with that letter, he will submit the past returns and other related documents. You have to submit the true name proof, and the company HR will handle the process. After that, you can claim 10-D. This will take some time.

As such, the name of the deceased is incorrect in the EPF records. Please approach your employer to fill out Form 20 and Form 10D, along with all other necessary forms for PF withdrawal. A clarification for the correct name of the deceased is required, as well as an undertaking from the dependent of the deceased.

Obtain a letter from the wife of the deceased requesting a correction in the name and forward the letter to the P.F. department along with your covering letter endorsing the correction based on the company's records. If the need arises, enclose a copy of the bank passbook of the deceased as evidence.

You have to fill out Form 20 for the EPF claim and Form 10-D for the pension claim. The nominee should produce a Legal Heir certificate given by the Tahsildar, mentioning the spouse and the children's names who are all under 18 (male) and if female, under 25. Bank accounts should be opened separately for all. Then the claim forms can be sent for processing.

If you know that the PF member's name is incorrect in the PF office records, you can produce an affidavit. Otherwise, you can provide his Date of Birth certificate from school or MC. Also, mention in your covering letter that due to an oversight, we had given the wrong name.
