Regarding this form, please include the following:
In Date / Out Date
In time / Out time
In Location / Out Location

Apart from that, I don't think you need this format at all when you are using the biometric machine. Even if employees forget the card at home, they can still punch the attendance via the machine.

Which machine are you using because nowadays, most biometric machines provide multiple systems to mark attendance, such as:
a) Punch Card/Swipe Card
b) Thumb
c) Pin (Just Like ATM Pin)

So, if an employee has forgotten/lost the card, they can still enter the PIN and mark their attendance. I have this kind of machine with me. Now, you can get all the exception reports, like who used the card, who used the PIN, and who used the thumb.

Do you really need formats for this? We just get an email, which the employee addresses to HR and marks a copy to his HOD, giving his IN/OUT time. Generally, he mentions "forgot swipe card". If this is frequent, one can investigate if there is a loss of card, etc.
