It was an interesting link, and it will be helpful in the future. Maybe I should tell you the background of my request. My organization implemented PMS in 1998, but it seems not as effective as expected. Support from the employees is very poor. Therefore, the management would like to conduct a survey to identify the underlying issues. Is it due to a lack of understanding, disbelief in the system, inadequate tools provided, or other reasons? I am supposed to start collecting questionnaires for the survey, but I am finding it difficult to formulate questions and answers. I need help with this.

Yep, I agree that it is more of a PMS survey. The management wants to get to the bottom of the problem. Since 1989, many programs have been introduced but lack support from the employees. Performance Appraisal Review (PAR), Performance Recognition In Development Enhancement (PRIDE), and many others seem to work. Therefore, it is decided to understand the problem. Is it because the tools and techniques are difficult? Or is it not communicated well, or people don't believe in it? Or is it because it is not linked to wages? Can you please help me?
