There is no NIL Increment format. But you may issue a letter as follows:

The management, with much regret, wishes to inform you that despite providing all support and facilities, your performance during the year 2009-10 was not satisfactory. Hence, you are not entitled to an increment this year.

You are further advised to improve upon your shortcomings (which have been communicated to you during the discussion), convert your weaknesses into strengths, and perform consistently well. This will make you worthy of receiving an increment next year.

Should you require any assistance or training to overcome your shortcomings, please feel free to express the same to HR.
